SANITAS will process your personal data in order to prepare a quote for the service, contact you by telephone, if you consider it appropriate, to answer your queries, to supply information on our services and our contractual obligations, and to provide assistance during the aforementioned process. The lawful basis for the processing is the implementation of pre-contractual measures, which you provide when you voluntarily fill in and submit this form. Your personal information will not be disclosed to third parties, unless there is a legal requirement to do so or it is necessary in order to provide our services. Similarly, for the purpose of completing the necessary processing, SANITAS may require other third-party service providers to access your personal data as data processors, subject to the appropriate guarantees.
To provide a quote for the service, we will process your personal data for a period of 10 days from your last interaction with SANITAS, unless we have your consent for further processing. After this period, we will store your personal data for the period of limitation for any liability that may arise as a result of the processing. You can exercise your rights of access, rectification, deletion, portability, limitation and opposition to the processing of your personal information, as well as your right not to be the subject of decisions based solely on the automated processing of your information and to withdraw your consent at any time, in writing and by post to "LOPD" to the aforementioned address, or by email to lopd@sanitas.es, attaching in both cases a document verifying your identity, if needed. You also have the right to file a claim with the relevant supervisory authority (www.aepd.es).
